Click on the “search” link above to view the available NDPHIT screening locations. Choose the one nearest you. You will be redirected to a page with information on the screening dates and location, as well as links to sign up and fill out your forms.

Attend your Biometric Screening! A trained medical employee will quickly test your blood glucose and cholesterol levels. You will then have a consultation with a physican to go over your results and identify any health risks. This short screening could potentially save your life!

Attend follow-up appointments with physicians to achieve and maintain your health goals. They will work with you as a mentor, motivator, and accountability coach.
